Job Description & Requirements

Experience Requirements

• Minimum: 2 years acute care hospital Case Manager experience
• Must Have: Care coordination & discharge planning experience
• Hospital experience is mandatory
• First-Time Travelers: Accepted (will be clinically screened)

Licensing

• State License: California RN license required
• Pending License: Accepted, but must be active by start date
