We are seeking a compassionate, dedicated Inpatient Transplant Coordinator to complete the following job duties with our inpatient liver transplant population.
Essential Duties:
-Responsible for the planning and coordination of all aspects of liver transplant patients' care from referral to post transplant.
-Serves as the primary coordinator for the interdisciplinary team, including physicians, nurses, social workers, dieticians, pharmacists, financial analysts, case managers, and support staff.
-Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community.
-Collects and documents data to meet clinical, financial, and regulatory requirements.
-Participates in the development of guidelines, protocols, pathways, and other performance improvement activities.
-Participates in research activities.
-Serves as a patient advocate throughout the transplant/surgical process.
-Represents the Medical Center at regional and national meetings/conferences.
Qualifications
Qualifications:
Education:
Graduate of an accredited nursing program required.
BSN required.
License/Certifications:
Current, unrestricted California RN License required.
BLS from the American Heart Association or American Red Cross required.
Specialty or Transplant certification required within one year from start date.
Experience:
A minimum of 3 years Acute Care or Critical Care experience required.
Experience as a Transplant Coordinator preferred.
